Do you need to provide photo copy of health insurance or just a number?

Our insurance agent requires that all participants provide a photo copy of their insurance card so we can provide it upon arrival at a medical treatment facility in the case of an emergency.  This will allow a camper to get the needed attention he requires without delay.

The safety of our campers is the most important part of our job and we do not take that lightly.

3. Email  PHOTO COPY OF FRONT AND BACK OF HEALTH INSURANCE CARD

What if i can't make the tryout date?

No problem, just sign up to be on the tryout roster and we will invite you to the first day of practice.  Following that practice, we will talk with you about your inclusion in the program.
